With Thanks, I humbly submit my career as a CHW (Community Health Worker) in the Beautiful City of Inkster, Mi, County of Wayne. A life that affords me the opportunity to not just advocate, advise and counsel Family and Friends where I once started, but also Community and Patients as I receive these same benefits in return. Diane works at the Cassopolis Family Clinic and has been a Community Health Worker for 5 years.
I am a certified Community Health Worker and work for Benzie-Leelanau District Health Department. I serve in Benzie, Grand Traverse and Leelanau counties. I am also a Public Health Tech and serve in many capacities as needed.
I am based out of Marquette County but cover the entire Upper Peninsula.

My main duties are helping clients find resources in my community, I keep up-to-date records for all resources. I also work directly with Grant agencies to maintain a fund for food and gas cards as needed, they also help with equipment, clothing, bedding or anything else that comes up including utility bills, purchasing a post office box and many more. I also go to the main food banks and pick up and deliver to client's home that do not have transportation. I take families shopping for children's clothing through another grant agency.
Started working for Upper Great Lakes Family Health Center in Gwinn, MI in 2016. Worked as a Care Manager for the Care Team and was in charge of the Breast and Cervical Cancer Navigation Program through MPCA and MDHHS. Then transitioned to doing Integration and being a Health Coach for Behavioral Health. Was a Care Manager for the SPIRIT study in coordination with the University of Washington and University of Michigan. Currently, I am a Behavioral Health Case Manager. My duties include managing counselor and MAT case loads. Addition responsibilities include coordinating and implementing a Youth SBIRT grant, STEPS study with the University of Washington, and a zero suicide curriculum grant.

She is also a strong community and social services professional with a Master of Science in Administration from Central Michigan University; and Master of Arts in Counseling (MA) focused in clinical mental health from Spring Arbor University.

At the Michigan Primary Care Association (MPCA), I work with MPCA’s Enabling Services network including supporting health center outreach, enrollment, CHW and other enabling services staff. I also take a very active role in our new partnerships with Medicaid health plans where health centers will be providing CHW capacity and services to support joint efforts/goals on patient engagement.

As Community Health Workers, we are able to assist individuals and families in linking with community resources and providing assistance in overcoming barriers in their lives. We are a listening ear and are dedicated to helping others to ensure an adequate resolution to their concerns. We assist the uninsured in applying for medical health insurance and promote programs that increase the quality of life of our community members.

I have only been a CHW for almost 1 year, started out doing home visits and helping clients, now I moved into a supervisor position and don't actually work with clients.
I enjoy helping people I have helped open a food pantry for our community. I'm working now with a doctor to open up a clinic.

I am a masters lever social worker, who has been working as a CHW supervisor for the last 5 years. During that time I have completed the CHW Certification Training and have obtained CHW Certification. I currently oversee 17 CHWs within our organization.
