Health Navigator (Highland Park)

Job description

The Health Navigator serves as a liaison, link or intermediary between health, social services and Ryan White eligible patients to facilitate access to services and improve the quality-of-service delivery and improve health outcomes like consistent engagement in HIV medical care, adherence to medications, and viral load suppression. This role also serves as an advocate for patients, a resource, and a vital part of the inter-professional healthcare team. Educates patients, engages, and assists them in managing their healthcare needs. Provides non-clinical paraprofessional duties and is thoroughly familiar with the patients’ community and the resources available. Collaborates with healthcare staff, community-based healthcare, social service, and other providers. *NOTE: This position specifically will begin working with the Ruth Ellis Health & Wellness Center and transition to working with Ryan White eligible residents of the Ruth Ellis Clairmount Center permanent supportive housing development in Detroit when it opens in Spring 2022 to support patients’ social determinants of health needs, resource navigation, and community integration needs, complementing the Henry Ford Health System clinical care team.

Education Requirements

High school diploma or GED equivalent required. · LGBTQ+ (Lesbian, Gay, Bisexual, Transgender and Questioning) cultural and diversity competency or lived experience with LGBTQ+ identification and/or homelessness. · Strong organizational and communication skills (written and oral). · Experience or familiarity with community support services. · Demonstrated professional maturity, responsibility, flexibility, and relationship building. · Familiarity with the Ruth Ellis Center’s principles of work: trauma-informed care, harm reduction, restorative justice, and positive youth development.
