Job description

Community Health Workers (CHWs) are frontline public health workers that serve as a liaison between the provider and health/social services. CHWs are seen as trusted members of the community by building a rapport with the community members that they are serving. Historically, CHWs have been successfully utilized to help manage chronic diseases, such as high blood pressure, diabetes, and high cholesterol. Provides social support and informal counseling, advocates for individuals, conducts basic health screenings, assists with referrals to a Primary Care Provider (PCP) and other community resources, and assists with enrollments in programs. Ensures successful linkage to care, the CHWs will follow-up with the participants to address any potential barriers to care

Education Requirements

High school graduate or equivalent. Valid driver’s license. Must have reliable transportation and insurance. Willingness to complete a formal Community Health Worker training program and achieve MiCHWA CHW certification within 12 months of hire date. Experience with public health work within the community preferred
