May 11, 2015 – Community Health Workers in Michigan – A Sustainable Approach
On May 11, 2015, 65 representatives from health plans and other agencies convened to discuss Community Health Workers (CHWs) in Michigan. The forum, Community Health Workers in Michigan – A Sustainable Approach, asked health plans and others to think about how CHWs could be a part of a new payment and service delivery system in the state. Participants heard background information on CHWs in Michigan and nationally and discussed needed policy components to sustain the profession. Attendees worked in small groups to brainstorm key points and considerations for CHWs in Michigan. This was the first health plan-focused convening around CHWs in Michigan. Timing was fortuitous, as the State of Michigan released its Medicaid Managed Care RFP May 8. The RFP contains a provision for CHW services.
June 11, 2015 – Sustaining Community Health Workers in Michigan: Taking Action for the Future
On June 11, 2015, 85 representatives from health plans and other agencies re-convened to discuss CHWs again. The forum, Sustaining Community Health Workers in Michigan: Taking Action for the Future, asked health plans, health systems, CHWs, and others to think about how CHWs could be a part of a sustained payment and service delivery system in the state. Participants again reviewed data and discussed sustainability in small groups. The result was a new list of discussion points as well as a draft action plan for CHW sustainability.
August 24, 2015 – Community Health Workers in Michigan: Next Steps
On August 24, 2015, 51 representatives from health plans and other agencies re-convened to get into the weeds on CHW payment. The forum, Community Health Workers in Michigan: Next Steps, asked health plans and some others to dive into payment models, CHW program supports, and CHW data. Four tables of participants discussed benefits and challenges of multiple payment models. One table discussed the needed infrastructure for supporting CHWs in programs, and the another table discussed the need for data to support the CHW. The result was a summary of small and large group discussion points, including recommendations for the continued work.
